LEAGUE RULES/FORMS

REGISTRATION FORMS - You may fill out these forms on-line and print them. All forms, except the Volunteer Form, are required to register a player. The Volunteer Form is needed for adult volunteers only.

Registration Form (please print four (4) copies for each player)

Medical Release Form (please print three (3) copies for each player)

Parent Code of Conduct (please print one (1) copy for each player)

Volunteer Form (needed for all adult volunteers - managers, coaches, team moms, umpires or volunteers with the Local League in any way)

In addition to filling out and submitting the forms listed above, you will also need residency documentation and an original or certified birth certificate to show proof of playing age for the season. Residency requirements include any correspondence received from a government entity, electric or water bills, a copy of a valid Texas driver’s license, voter’s registration cards, etc. All residency requirements must have a physical address; not a post office box and both proofs must be different. For example, a player cannot present both his mom’s and dad’s driver’s license as proof of residency. However, a player can present his mom’s driver’s license and an electric bill for his family. Any correspondence must be addressed one of to the parents and cannot be addressed to the minor player. 2005’s requirement (for the All-Stars) was three proofs of residency.
